What to do

  1. Check if your kit matches one of the affected lot numbers: 25AMD225, 25BMA495, 25GMD834, 25HMJ685, 26AMD781, 26DMI370, 26FMG952, or 26FMJ739.
  2. Do not use applicators from affected lots if the packaging appears open or damaged.
  3. Contact Medline or your healthcare facility for instructions on replacement or return of affected kits.
  4. If a potentially contaminated applicator was used, inform your healthcare provider.
