The rails also lack required warning labels and have other structural and design flaws that create fall, laceration, and entrapment hazards.

The drive converter sheave in certain 2026 Kawasaki Teryx4 H2 and Teryx5 H2 off-road vehicles can break during operation, releasing metal fragments into the engine compartment. Kawasaki has received 19 reports of this issue, though no injuries have been reported yet.

The passenger-side brake cable on certain Yamaha Drive2 Electric Fleet Golf Cars (model years 2021–2022) may have been installed incorrectly during manufacturing. This misrouting can cause the cable to wear abnormally and break during use, which could cause a loss of braking control and serious injury or death.

The plastic brackets and straps can break or degrade over time, which could cause furniture to tip over. No injuries have been reported, but a broken restraint could create a serious fall hazard, especially for children.

The Merkury Innovations Hot + Cool Heating and Cooling Fan (model MI-DHC02) can overheat and catch fire when plugged in. The company has received two reports of fires, including one that caused smoke damage to a home. No injuries have been reported so far.

LiKee Pull String teething toys have silicone strings that are longer and smaller than safety standards allow. The strings can reach the back of a child's throat and become lodged, causing choking or difficulty breathing. The company has received ten reports of children experiencing respiratory distress or choking from these toys.

Metal springs in the detachable hinged lids of Kitchen HQ brand thermal insulated bowls can catch fire when used in the microwave. HSN has received 30 reports of the bowls smoking, sparking, melting, or catching fire, including one that caused property damage.

Joyin's Sloosh dive sticks (model 40041) are too hard and can cause serious piercing injuries if children fall or land on them in shallow water. Facial and eye injuries are also possible when retrieving sticks underwater. These sticks violate federal safety standards for dive toys.

The CNG tank support bracket in certain Cummins Clean Fuel Technologies 75DGE fuel systems can become deformed during a side-impact crash, potentially causing the tank to dislodge or detach. A loose tank can leak fuel, creating a fire risk.

The fan blade in Vornado SRTH tower heaters can detach from the motor shaft, causing the fan to slow or stop. This leads to overheating and melting of internal parts, which can ignite and cause a fire if the safety cutoff fails. Vornado has received 32 reports of overheating, including 8 fires and 1 smoke inhalation.

The SUNS SS6291 interlock switch, used in residential elevators, can get stuck in a retracted position. When this happens, the exterior elevator door may stay unlocked even when the elevator moves to another floor, creating a risk of falling or being crushed.

The hard plastic outer shell on these bottles can bubble or peel, creating loose pieces of film-like plastic that a baby could choke on. The manufacturer has received 135 reports of this issue, though no injuries have been reported yet.

Broqixin-branded pool drain covers do not meet federal safety standards and can trap swimmers underwater, creating a drowning hazard. The white plastic covers (Model 11064W, about 7.36 inches wide) were sold online through Amazon from January to April 2026.

Gudook-branded bicycle helmets (model KY-055) do not meet mandatory safety standards for impact protection. The helmets can fail to protect your head in a crash, creating a serious injury or death risk. Only helmets with batch number 202503 and manufacture date March 2025 are affected.

Crenlux pool drain covers do not meet federal safety standards and can trap swimmers or bathers underwater, creating a serious drowning hazard. The white plastic covers, about 8 inches in diameter, were sold online through Amazon from June 2025 to March 2026.

Tiny Land children's play tents contain fiberglass poles that can shed fibers, causing skin and eye irritation. The company has received eight reports of fiber shedding resulting in irritation. The tents were sold online from November 2025 through March 2026.

Little Grape Land nursing pillows sold on Amazon do not meet federal safety standards and can obstruct an infant's breathing. The U-shaped pillows, sold in various patterns between August 2025 and April 2026, pose a serious risk of suffocation during infant feeding or tummy time.

Wyze Solar Cam Pan security cameras can overheat and catch fire if installed incorrectly. The incorrect assembly instructions may lead users to accidentally puncture the lithium-ion battery's metal casing with a long, flat-head screw, causing the battery to rapidly overheat. The company has received reports of six cameras catching fire and exploding, with six consumers suffering minor burns.

Mainstays 9-drawer fabric dressers sold at Walmart can tip over if not anchored to the wall, creating a risk of serious injury or death to children. The dressers do not meet federal safety standards for clothing storage units. About 165,000 units are affected.
